Results from the project development were presented at VAKE´s HYVAt messut event

Employees in the three-year VAKEhyva – Hyvät palvelut project presented successes and results at the HYVAt Messut event in April. The goal with developing services and operating models has been, to give clients faster access to care and support at the right time.

Participants in VAKE´s HYVAt messut event were able to learn about the results from VAKE´s development work.

Services have been developed in the wellbeing services county of Vantaa and Kerava (VAKE) in the VAKEhyva – Hyvät palvelut project, that started in 2023 and is funded by EU. The goal with the development has been to improve the clients access to services and strengthen prevention and identification of problems for those residents who are in a weak and vulnerable position. The projects will end in the end of 2025.

Project workers presented at the HYVAt Messut event held 11.4.2025 the final results for the project to employees in the wellbeing services county and operating models developed together with sectors in VAKE, which improve access to services and accessibility of services.

Multilingual child families access to services - pilot project

In the operations the focus is on improving accessibility to services for multilingual child families, who doesn´t have Finnish or Swedish as mother tongue.  The development work started in January 2024 and its goal is to increase the awareness about services for children, young people, and families, lower the threshold for accessing services and reduce the fear of authorities.

Modell for developing MyTeams for oral health care

The goal for the development is to improve access to oral health care services and the continuity of care especially for those patients, for whom the continuity of care is especially important. The introduction of MyTeams has been prepared since spring 2024 with participatory development and MyDentist–MyDentalHygienist pilot project.

Model for doctor’s work given at home for informal care families with older people

This service aims to anticipate changes in clients and patients’ wellbeing, strengthen holistic health and wellbeing and support possibilities to live at home. 41 families participated in the pilot project, where the informal carers are mainly old persons and take care of their spouse at home. The service includes home visits of doctor and nurse, support of a nurse by telephone as well as the possibility to consult a doctor.

Start to talk about wellbeing and service guidance

The aim with the start to talk about wellbeing and service guidance is to get a more holistic picture of the client’s situation, so he/she can find suitable services also offered by the cities and organizations. You can look after services in the Wellbeing Services of Vantaa and Kerava.

Registration practices in social and health care as well as structural social work

The goal with the development task has been to standardize the information production and improve the use of national registration instructions. VAKES own networks for developing registration formed during the project as well as take into use the tool for social reporting produced by Centre of excellence on social welfare Socca.
