At the Senior Counselling Service, you will be assisted by a healthcare and social welfare professional. The conversations are confidential.
The Senior Counselling Service will provide you with advice and support on everyday issues and help you according to your needs. If necessary, you will be referred to other services.
You can always get in touch again if you have new questions. A home visit from a service coordinator may be organised if your situation needs further investigation.
The Senior Counselling Service will book you an appointment for a service needs assessment if your situation requires it.
The service needs assessment is usually carried out in your own home.
Your loved ones may be present at the meeting if you so wish. The presence of your relatives often helps to gain a better understanding of your situation.
At the meeting, the service coordinator will discuss your life situation and resources with you. Together, you will consider which activities or services could contribute to your well‑being and everyday life.
You will receive guidance and advice during the meeting. If necessary, services to support your living at home will be initiated.
Services that support living at home are granted in accordance with the criteria for granting services. You can find more information about the service needs assessment on the Senior Counselling Service website.
After the assessment, we will make the necessary service decisions and the agreed services can be initiated.
An individual client plan will be drawn up for you based on the service needs assessment. The plan will outline the services and support measures you need.
Services may be provided by the wellbeing services county, private operators, organisations, municipalities or churches. You can also purchase services yourself if you wish.
We will regularly monitor your situation and the services you receive. We will update your client plan if your situation changes.
If your situation changes and you are already receiving services, you can contact the service provider, such as the home care unit, directly.
We grant services to older people according to the grounds for the granting of services.
