The service is intended for clients of the wellbeing services county's health services. It is provided based on a health care professional's assessment of the need for treatment and its urgency.
The At-home Hospital is located on the 5th floor of the Plaza Business Park premises, in the Hehku building.
It provides 24-hour care in the patient's home, service home or nursing home for Vantaa and Kerava residents over the age of 16. Non-resident patients are treated with a payment commitment, and each situation is assessed on a case-by-case basis together with the doctor.
At-home hospital care provides short-term care or support after prolonged hospitalisation. You need a doctor's referral for the service. You must also be able to cope with everyday activities independently or with the support of loved ones/home care and be committed to the treatment.
After the need for assistance has been assessed, the home visits are planned together with the patient and the necessary support services are arranged. The treatment period includes assessment/visits by a nurse and doctor. Patients can contact the At-home Hospital around the clock.
The At-home Hospital treats, provides support and takes care of, for example: - patients with infectious diseases and patients in end-of-life care at home - medication, wound care and laboratory monitoring - symptomatic treatment of cancer patients - daily activities of the client
