Acute Geriatric Ward 2 is part of the Vantaa and Kerava welllbeing services county's hospital services, located at Peijas Hospital, and it operates together with the At-home Hospital. The ward has 28 beds for Vantaa and Kerava residents over the age of 55 who have an acute illness treated by primary health care.
Patients come to the ward via Peijas Hospital's emergency department around the clock. Our aim is to discharge the patients as soon as possible, and we work together with other actors …involved in the patient's care and rehabilitation.
Each patient is assigned a primary nurse who handles all the patient's affairs: looks after them while at work, deals with other staff groups on the patient's behalf or with the patient, and communicates with the patient's relatives when necessary. In all treatment-related matters, the primary nurse is the first point of contact.
The aim of the treatment and rehabilitation is to maintain and promote your functional ability. The emphasis is on your personal goals, wellbeing and holistic rehabilitation approaches that allow you to participate in society. You are involved in setting your own objectives as well as planning the care and rehabilitation period together with the hospital's multidisciplinary team and your loved ones. The achievement of the objectives is assessed periodically together with the team.
